Position Description:The Assistant Wellness Director (AWD) position will assist the Wellness Director to maintain department of health regulatory compliance and coordinate all services for Independent or Enriched Housing residents provided by Hearth Management into a cohesive system to maintain a continuing awareness of each resident-s total well-being (social, family, financial, mental health, key life events, dietary needs, spiritual needs, health and medical needs; including, but not limited to medications and personal care needs). They will maintain their assigned case load of residents and act as a reference for other Case Managers while preforming quality assurance auditing practices to maintain compliance.
Education/Training/ Experience:An Associate-s degree from an accredited college or university with major work in human services or service delivery to include an LPN or RN degree and three years full-time experience in the provision of services to a frail adult population. Flexibility in work schedule is key to respond to urgent issues as they arise after hours. Must have advanced working knowledge of state department of health regulations. Requires excellent Verbal, Written and Interpersonal Skills. Proficiency in Microsoft Office applications such as Word, Excel and Outlook.
