Job Description:
AccentCare is seeking a compassionate and professional
Registered Nurse to join our hospice care team. Our nurses are
committed to providing empathetic, comprehensive end-of-life care
that respects the dignity and personal needs of each patient. As
part of our dedicated team, you will offer skilled nursing care in
home settings, working collaboratively with patients, families,
physicians, and other team members to ensure an effective and
comforting hospice experience. Your role is pivotal in helping us
fulfill our mission to enhance the lives and well-being of our
patients through personalized, high-quality care. Provide direct
patient care to hospice patients, following a specific care plan
tailored to individual needs. Monitor patient health conditions,
administer medications, and document patient's health progress.
Educate patients and their families on end-of-life care and comfort
measures. Coordinate with physicians, hospice team members, and
other healthcare professionals to optimize care plans. Respect
patient dignity and confidentiality, ensuring a compassionate
environment at all times. Strong interpersonal and communication
skills Ability to work compassionately with patients and families
Advanced knowledge of nursing practices and palliative care
Proficiency in documentation and patient management software
Medical Insurance Dental Insurance Vision Insurance 401k Valid RN
license in the state of employment BLS Certification Preferred:
Certification in Hospice and Palliative Nursing (CHPN)
