Job Description:
AccentCare is seeking a dedicated Registered Nurse (RN) to join
our Hospice team, providing end-of-life care to patients in their
homes. With a focus on compassionate, personalized care, the ideal
candidate will possess a deep understanding of pain and symptom
management in order to enhance patient comfort, dignity, and
quality of life during their final stages. Through tailored
healthcare plans and working closely with patients' families, our
nurses offer crucial support during challenging times. This role
involves administering medications, performing assessments,
coordinating with healthcare providers, and above all, ensuring a
respectful and supportive environment. Manage patient care with
specific knowledge and experience in bedside nursing, symptom
management, crisis intervention and family intervention. Administer
medications and treatments as prescribed by the physician in the
patient's plan of care. Assess the patient’s condition during every
visit and chart your observations. Educate patients and their
families on proper home health care strategies and procedures.
Coordinate with a team of healthcare professionals to share
expertise and knowledge. Offer emotional and psychological support
to the patients and their families. Advanced knowledge of
end-of-life care and pain management. Strong interpersonal,
communication, and organizational skills. Ability to handle
emotional and stressful situations. Medical Insurance Dental
Insurance Vision Insurance 401k Current RN license in the state of
practice. CPR certification. Hospice and Palliative Nurse (CHPN)
certification preferred.
