Job Description:
Signify Health, a pioneer in leveraging technology and advanced
analytics to foster value-based healthcare, is seeking a dedicated
Nurse Practitioner for our In-home Visits team. This role involves
providing high-quality, direct healthcare services to patients in
their homes, addressing the needs of those who might otherwise have
difficulty accessing healthcare facilities. You will play a
critical role in our strategy to deliver personalized, on-site care
that both respects patient comfort and contributes significantly to
holistic health outcomes. Conduct comprehensive assessments
including health history, physical examination, and evaluation of
patient needs within their home environment. Develop and implement
care plans tailored to individual patient needs in collaboration
with a multi-disciplinary team. Prescribe medications and recommend
lifestyle adjustments or other therapies. Provide guidance on
health maintenance and disease prevention. Document findings and
care provided in the patient's medical record accurately and
timely. Follow up with patients to assess progress and adjust care
plans as needed. Collaborate with primary healthcare providers and
other health professionals to ensure continuity of care. Advanced
diagnostic and clinical judgement capabilities Excellent
interpersonal and communication skills Ability to work autonomously
in a non-clinical environment Proficiency with health IT systems
and mobile health applications Malpractice coverage Valid Nurse
Practitioner license Board certification from the American Nurses
Credentialing Center (ANCC) or the American Association of Nurse
Practitioners (AANP) Valid driver’s license and reliable
transportation as this position requires travel to patient
homes
