We are bold innovators, dedicated to making a difference and we hire wicked smart people We are looking for an Clinical Support Coordinator.Job Summary: The Clinical Support Coordinator (CSC) is responsible for the support of clinicians performing in-home and telehealth assessments. CSCs work in conjunction with the Member Engagement team to analyze market data, review clinician scheduling, assist in monthly forecast production, and identify trends among clinicians to present information for business and operational planning. He/She will work to ensure InSight clinicians are efficiently scheduled, minimize travel time of the provider throughout each market, support. CSCs ensure that our field clinicians are set up for success. Primary Responsibilities:Communicating with Provider on status of case submission in the PopHealthCare portal.Coordination of requested supplies. Maintain supply chain and inventoryCoordination and communication of weekly/monthly schedules.Coordination and communication of coverage areas.Daily communication/touch base with Providers/clinicians on scheduled work days.Coordination of Clinical and Coding Program Training with clinical operations.If during a reminder call, a member will not be available at the scheduled time, the Clinical Support Coordinator will contact the scheduling center so that every effort can be made to fill that time slot.Communicating with Provider daily on status of the daily scheduled exams. The provider must report in the daily completion status of scheduled exams by either a phone call or email.Review reports for slot management, clinician availability, missing exams, patient health profile, etc.Participate in required weekly operation team meetings and monthly provider meetings.Complete and submit Termination Request Form to HR when provider resigns or terms.All other duties assigned by management.QualificationsHigh School Diploma / GED or higher Associates Degree (or higher) in healthcare, business or other related field preferredPrevious experience in healthcare operations, workforce management or data analysis 6+ months of Microsoft Excel experience working with Pivot Tables, logical functions, Previous experience in data modeling, trending and forecasting a plusExcellent written and communication skillsPopHealthCare is an equal opportunity/equal access employer fully committed to achieving a diverse workforce.
