Responsibilities:
At Fast Pace Health, the Population Health Advocate provides
outstanding customer service and maintains a compassionate and
welcoming atmosphere. The Population Health Advocate provides
effective customer service by exploring the needs and priorities of
patients and office offerings. The Population Health Advocate will
provide effective customer service for all internal and external
customers by using, excellent, in-depth knowledge as well as
communicating effectively with team members. The Population Health
Advocate will develop trusting relationships with the patients and
be the primary contact for your assigned patients. The ideal person
will have understanding and strong flexibility in developing a
consistent approach for excellent patient care. Strong
collaboration will be necessary, with direction and guidance coming
from various members of the population health teams, with the goal
of patient excellence.Below, we have provided a high level view of

in detail during our interviewing process.Identifies patient care
requirements and goals by establishing personal rapport with
patients and other persons in a position to understand care
requirements.Establishes a compassionate environment through
interactions with patients and caregivers.Documents patient
interactions in the electronic medical record (EMR) and other
software platforms.Monitors clinical status of patients (e.g.,
medication adherence and referral follow-ups) through dedicated EMR
and software. Maintains continuity of care by documenting and
communicating actions, responds timely to requests, builds and
maintains rapport with the clinical team, escalates issues to day
to day supervisor as well as Director of Quality as needed. Abides
by HIPAA and maintains patient information confidentiality and
privacy.Assists physicians and clinicians in orienting patients to
their care plans and the Fast Pace Health network.Problem solves
questions from patients or staff (e.g. problems with pharmacy,
insurance, prior authorizations).Assists with specialty projects
pertaining to the overall improvement and quality of
care.Identifies and socializes opportunities to improve care
journeys for Fast Pace patientsReports to work location as directed
and is willing to physically work onsite at the assigned clinic
when provider is present to provide real-time assistance to
patients for identified care needs.Maintains safe and clean working
environment by complying with procedures, rules, and regulations;
calling for assistance from health care support personnel.Supports
patients in decision making related to healthcare needs. May make
suggestions for improvements & recommends solutions that fit the
patient's needs. Demonstrates superior product knowledge;
accurately describes the features & benefits of all products using
the appropriate tools.May serve as primary point of contact for
members as they engage the service - this includes inbound and
outbound.Educate member on available services within their
respective health plan to meet their health related needs.Assists
patients with scheduling follow-up visits and wellness exams.
Communicates this information to their care team - including
primary care, specialty care, and urgent care providers.Assists
patients with medication adherence via reminders and best
practicesConnect patients with community resources as
needed.Responsible for coordinating delivery date and time, set
expectations for pick up, seeks feedback about experience and asks
for referrals. Responsible for learning the aspects of compliance
in the company by completing all mandatory compliance trainingAble
to build trust and maintain friendly, cordial relations with all
employees and patients by acting and communicating in a manner that
results in a positive work relationship with customers, co-workers
and managers.The ability to perform the physical, use of senses,
cognitive, and environmental functions of the position, as
specified on the physical demands.Ability to comply with Company
standards of operations and adhere to the Core Values of the
Company, of teamwork, communication, empowerment, quality of care,
and friendliness.
Experience Requirements and Preferences::
Education: High School Diploma or it's equivalent; Coursework or
experience in Psychology, Social Work, Nursing or related allied
health field are helpful AND Experience: At least 1 years of
experience in a sales, customer service or health promotion role
preferably in a medical setting. Experience preferred in promotion
of healthy living (health care, hospitals, wellness programs,
etc)
